Services provided on a sliding fee scale, and income verification is required.  Enrolling takes about 30 minutes - for Hawaiians and their `Ohana.  No blood quantum.

Please bring the following items with you:

  • A copy of your birth certificate that states Hawaiian ethnicity (if applicable)
  • A copy of your insurance cards if you have insurance
  • A copy of your families past two-month income stubs (Paycheck, unemployment stubs, social security, welfare financial sheet, etc...)
